OSHA 300 Log Workbook: Excel Log, 300A Summary, TRIR and DART Calculator, and Deadline Calendar
A working Excel workbook for small employers who keep their own OSHA injury and illness records.

What it does
Log each case once, in the Form 300's column order, and the workbook fills the death / days away / restricted / other columns for you, prints a Form 300 layout with privacy cases masked, totals the year into a Form 300A summary, works out your TRIR, DART rate and days-away case rate from your hours, and dates the certify-and-post, submission and retention deadlines for your year. A five-year trend sheet charts your rates. Every figure is a live formula you can inspect, and the formulas follow OSHA's forms package and BLS's incidence-rate method, the same sources the free calculators on trircalc.com are checked against.
What's included
- Setup sheet: annual average employees from a 52-row pay-period table, hours from payroll or OSHA's estimate worksheet, full-time equivalents
- Log sheet: dropdowns, one outcome per case, and a Check column that flags missing data, days that don't fit the outcome (Form 300's most-serious-outcome rule), dates outside the Setup year, and counts past 180 days
- Form 300 View: print-ready landscape layout, privacy cases shown as "Privacy Case"
- Form 300A: case totals, type totals with a match check, rates, establishment block, certification block
- Calendar: February 1 certify and post, March 2 submission, April 30 take-down, five-year retention, with done tick-offs and days-left; 8-hour and 24-hour event clocks
